This position will be in our In Home Service department of Breakthrough In Home Service. We are looking for skilled professionals to provide services in the comfort of the client’s home.

Responsibility:

·      Patient assessments

·      Health monitoring

·      Training of family members to perform some procedures

·      Homemaker chores

·      Advance personal care

·      Respite

Requirements:

·      Age 18 and over

·      Current, unencumbered state certification to practice as a Certified Nursing Assistant (Required for CNA’s Only)

·      Must have a valid Driver’s License, State ID or Passport

·      Must have Social Security Card or Birth Certificate

·      Must be registered on the Family Care Safety Registry (FCSR) - $15.25 fee (Cash or Money Order Only)

Education/License:

CNA, CPR Certified

Experience:

Minimum of 6 months experience required
